Below is a matrix that will serve as a checklist for each of the four different IWS Thyroid Study options.
Please read through this sheet and all the forms before going to your Vet's office! Make sure that you bring Vet Instructions and all the completed necessary forms with you to your Vet's office when you go to have your dog's blood drawn so that they can be packed along with the blood samples.
